Tesamorelin 10 to 20 mg

GHRH analogue · FDA approved in 2010, but only for HIV-related abdominal fat, at 2 mg daily

A synthetic analogue of human growth hormone-releasing hormone (GHRH). It is not growth hormone itself: it stimulates the pituitary to release GH in a pulsatile pattern, and that GH raises IGF-1 in the liver. It carries the complete 44-amino-acid sequence of human GHRH with a hexenoyl chain attached to the N-terminal tyrosine, a minimal change that makes it resistant to the DPP-4 enzyme and prolongs its action compared with natural GHRH. It is marketed as Egrifta and Egrifta SV (Theratechnologies). The FDA approved it in November 2010 ONLY for reducing excess abdominal fat (visceral fat) in adult HIV patients with lipodystrophy, at a dose of 2 mg subcutaneous daily (Egrifta, 1 mg vial, two vials) or 1.4 mg daily (Egrifta SV, 2 mg vial, discarding the leftover). The FDA's own label states it is not indicated for weight loss, that it has a weight-neutral effect, and that its long-term cardiovascular safety has not been established.
